KEEP FIT LOG FORMS

KEEP FIT offers the below forms to submit your exercise and nutrition information to your trainer. Please use the form as directed by your trainer and try to fill in ALL of the fields. Talk to your trainer if you have any questions regarding your submission.

The KEP FIT logs are available in two file formats: MS Excel and Adobe PDF format.

Use the Adobe PDF format to print, fill manually and fax to KEEP FIT. This will require Adobe Acrobat reader. Click here to download a free copy!

If you have Microsoft Excel, you may use the Excel format to fill the form electronically and mail back to george@keepfitclub.com

To download and save, right click on the preferred format and select "Save Target As"

FITNESS LOG GUIDE:
  • PROTEINS = Egg Whites, Filet Mignon, Turkey, Chicken, Fish, Deer, Lamb.
  • STARCHY CARBS = Red Sweet Potatoes, Brown Rice, Whole Grains, All Beans, Black, Red, Navy, Pinto ETC.
  • VEGGIE FIBERS = Broccoli, Asparagus, Brussel Sprouts, Cauliflower, Spinach, Swiss Chard, Kale, Green Beans.
  • FRUITS = All Berries, Cherries, Apples, Pears, Grapefruit, Kiwi.
  • FATS = Oils- Olive & Sunflower, Nuts- 1 OZ Of Almonds, Walnuts, 1 Egg Yolk.
  • DAIRY = Skim Milk Is 10% Fat, 1% Milk Is 17% Fat, 2% Milk Is 34% Fat, Whole Milk Is 50% Fat, Cottage Cheese-1 or 2%.
